One of the most common things I hear from patients is:
“My neck isn’t too bad, but my shoulder has been killing me ever since the accident.”

If you’ve recently been involved in a car accident in Bellevue, Seattle, Redmond, Kirkland, Issaquah, or the surrounding Eastside, you’re not alone. Shoulder pain is incredibly common after a collision—even in what seems like a minor fender bender.
The frustrating part? Your shoulder may feel perfectly fine immediately after the crash and only begin hurting hours or even days later.

Why Does My Shoulder Hurt After a Car Accident?
During a collision, your body experiences forces it was never designed to handle.

Common Shoulder Injuries After a Car Accident

Rotator Cuff Strain or Tear
Your rotator cuff consists of four muscles and tendons that stabilize your shoulder.
Symptoms include:
  • Pain reaching overhead
  • Weakness lifting objects
  • Difficulty sleeping on the injured side
  • Pain putting on a jacket
  • Sharp pain reaching behind your back

Shoulder Sprain
The ligaments surrounding the shoulder can become overstretched during impact.
Symptoms include:
  • Swelling
  • Aching pain
  • Limited range of motion
  • Pain with movement

AC Joint Injury
The acromioclavicular (AC) joint sits on top of the shoulder.
Seatbelt trauma or direct impact can injure this joint, causing:
  • Pain on top of the shoulder
  • Tenderness to touch
  • Pain lifting your arm
  • A noticeable bump in more severe injuries

Labral Injury
The labrum is a ring of cartilage that helps stabilize the shoulder.
Damage may cause:
  • Clicking or popping
  • Catching sensations
  • Deep shoulder pain
  • Instability
  • Weakness

Muscle Strain
Muscles surrounding the shoulder blade often tighten dramatically after a collision.
These muscles frequently become painful several days after the accident and can cause:
  • Knots
  • Burning pain
  • Pain between the shoulder blades
  • Limited movement
  • Headaches

Sometimes the Shoulder Isn’t Actually the Problem
One of the biggest surprises for many patients is that shoulder pain isn’t always caused by a shoulder injury.
The nerves that supply the shoulder originate in the neck.
Whiplash injuries frequently irritate these nerves, creating pain that radiates into the shoulder, upper arm, or even the hand.
This is why a proper examination is so important.
Treating only the shoulder while ignoring an injured neck often leads to slow recovery and lingering pain.

Why Didn’t My Shoulder Hurt Right Away?
Adrenaline is incredibly powerful.
Immediately after a crash, your body releases stress hormones that temporarily block pain.
Once those chemicals wear off, inflammation begins and injured tissues become painful.
It’s common for shoulder pain to appear:
  • Several hours later
  • The next morning
  • Two or three days after the accident
Never assume you’re uninjured simply because you felt okay at the scene.

Should I Get Checked Out?
Yes.

Even if X-rays show no broken bones, soft tissue injuries such as muscle strains, ligament sprains, tendon injuries, and whiplash will not appear on standard X-rays.
A thorough examination helps determine:
  • Whether your pain is coming from the shoulder or neck
  • Which structures are injured
  • Whether advanced imaging may be needed
  • The best treatment plan for your recovery
